Welcome to the website of Power Wellness Health PC ("Power Wellness," "we," "us," or "our"). These Terms and Conditions govern your access to and use of our website, patient-facing online services, scheduling tools, membership services, telehealth services, and related offerings.
By accessing our website, scheduling an appointment, enrolling in a membership, submitting payment, or using our services, you agree to these Terms and Conditions. If you do not agree with these terms, please do not use our website or services.
1. Medical Practice and Services
Power Wellness Health PC provides healthcare and wellness services through appropriately licensed medical professionals. Services may include health evaluations, laboratory review, hormone optimization, medical weight-management services, wellness consultations, regenerative medicine services, and other medically appropriate services.
The availability of any treatment, medication, procedure, or service depends upon an individualized medical evaluation. No treatment or prescription is guaranteed.
Services are currently available only to patients physically located in Pennsylvania at the time medical services are provided, unless Power Wellness expressly confirms otherwise and the treating provider is properly authorized to practice in the applicable jurisdiction.
2. No Medical Advice From Website Content
Information presented on this website is provided for general educational and informational purposes only. Website content does not constitute individualized medical advice, diagnosis, treatment, or a substitute for an evaluation by a qualified healthcare professional.
Viewing this website, submitting a contact form, communicating with administrative staff, or using general educational resources does not by itself establish a provider-patient relationship.
A provider-patient relationship is established only after all required registration materials and consents have been completed and a licensed provider has agreed to provide care.
3. Medical Emergencies
Power Wellness does not provide emergency medical services through this website, email, text messaging, patient portal, or telehealth platform.
If you believe you are experiencing a medical emergency, call 911 or go to the nearest emergency department immediately. Do not rely on website communications, portal messages, email, or text messaging for urgent or emergency medical needs.
4. Telehealth Services
Certain services may be provided through telehealth. Telehealth may involve electronic communications, video conferencing, remote review of medical information, and other technology-assisted interactions.
Telehealth has potential benefits and limitations. Technical interruptions, unauthorized access, delays, or other technology-related issues may occur. Patients are responsible for using a reasonably private location and secure device when participating in telehealth services.
Before receiving telehealth care, patients may be required to complete separate telehealth, treatment, privacy, and informed-consent documents.
5. Eligibility and Patient Responsibilities
By requesting or receiving services, you represent that:
- The information you provide is complete, current, and accurate.
- You will disclose relevant medical conditions, medications, allergies, symptoms, and health-history information.
- You will complete requested laboratory testing and follow-up appointments.
- You will follow treatment instructions and promptly report material side effects or changes in your condition.
- You will not provide false information, impersonate another person, or obtain services or prescriptions for another individual.
- You are legally authorized to enter into these Terms or have the consent of a parent or legal guardian when required.
Failure to provide accurate information or comply with clinical requirements may result in treatment being delayed, changed, or discontinued.
6. Prescriptions and Pharmacy Fulfillment
Prescription medications are prescribed only when a licensed provider determines that they are medically appropriate following a proper clinical evaluation.
Power Wellness does not guarantee that a particular medication will be prescribed. Prescription decisions remain within the independent professional judgment of the treating provider.
Prescriptions may be transmitted to an appropriately licensed third-party pharmacy. Medication availability, formulation, processing time, shipping, packaging, and delivery are subject to the pharmacy's operations and applicable law.
Certain medication and pharmacy costs may be included within a patient's program or membership price. Power Wellness may pay a pharmacy or other third-party provider as part of fulfilling the patient's treatment plan. Unless specifically stated otherwise, the patient is purchasing medical services and associated program benefits from Power Wellness.
Third-party pharmacies, laboratories, shipping providers, and other vendors may maintain their own terms, policies, and delivery requirements.
7. Laboratory Testing
Laboratory testing may be required before treatment begins and periodically during treatment. Patients are responsible for completing requested testing within the required timeframes.
Depending on the service and laboratory used, laboratory costs may be included in a program price, billed separately by Power Wellness, billed by the laboratory, or submitted to the patient's insurance.
Insurance coverage is not guaranteed. Patients are responsible for confirming coverage and for any deductibles, copayments, coinsurance, denied claims, or other amounts not paid by insurance.
8. Fees and Payment Authorization
Fees for appointments, programs, procedures, memberships, laboratory services, medications, supplies, and other services will be disclosed before purchase or enrollment when reasonably practicable.
By providing a payment method, you represent that you are authorized to use it and authorize Power Wellness and its payment processor to charge all amounts that you have agreed to pay.
You are responsible for maintaining accurate payment information. Declined or failed payments may result in delayed services, suspension of membership benefits, cancellation of appointments, or interruption of treatment.
Charges may appear on your card or bank statement under the billing descriptor "Power Wellness" or a substantially similar descriptor.
9. Memberships and Recurring Payments
Certain Power Wellness services are offered through recurring membership or treatment programs.
By enrolling in a recurring program, you authorize Power Wellness to charge the payment method on file at the frequency and in the amount disclosed during enrollment. Charges will continue until the membership is properly canceled in accordance with the applicable membership agreement and cancellation policy.
Unless different terms are stated in a signed membership agreement:
- Memberships may require an initial six-month commitment.
- Memberships may automatically renew after the initial commitment period.
- Cancellation requests must be submitted at least 30 days before the applicable renewal or billing date.
- Canceling a membership does not automatically refund charges already processed or amounts owed under an existing commitment.
Any separate membership agreement signed by the patient will control if it conflicts with this general section.
10. Appointment Cancellations and No-Shows
Patients must provide at least 24 hours' notice when canceling or rescheduling an appointment.
Appointments canceled with insufficient notice and missed appointments may be subject to a fee of up to $100, except where prohibited by law or waived by Power Wellness under appropriate circumstances.
Repeated missed appointments may result in limitations on future scheduling or termination of the provider-patient relationship, subject to applicable professional requirements.
11. Late Payments
Amounts not paid when due may be subject to a late fee of up to $50 after five days, to the extent permitted by law and disclosed in the applicable patient or membership agreement.
Patients remain responsible for outstanding balances, including balances associated with completed services, ordered products, medications, laboratory testing, or contractual membership commitments.
12. Refunds
Medical evaluations, consultations, completed appointments, laboratory services, procedures, administrative services, and other services that have already been provided are generally nonrefundable.
Prescription medications, compounded medications, temperature-sensitive products, customized products, and products prepared or ordered specifically for an individual patient generally cannot be returned or refunded once ordered, dispensed, shipped, or delivered, except when required by law or when Power Wellness or the dispensing pharmacy determines that an error occurred.
Requests involving damaged, incorrect, or undelivered shipments must be reported promptly so that Power Wellness can coordinate with the applicable pharmacy or shipping provider.
Any approved refund will be returned, when possible, to the original payment method. Refund timing may depend on the payment processor and financial institution.
Additional refund provisions contained in a signed membership, treatment, or procedure agreement will also apply.
13. Treatment Outcomes and Risks
Healthcare outcomes vary among individuals. Power Wellness does not promise or guarantee a particular health, cosmetic, weight-loss, hormonal, performance, or wellness outcome.
All medical treatments, prescriptions, supplements, and procedures involve potential risks, side effects, contraindications, and limitations. These will be addressed through the applicable clinical and informed-consent process.
Testimonials, reviews, educational materials, before-and-after examples, or descriptions of potential benefits do not guarantee that another patient will experience the same result.
14. Communications
By providing your telephone number or email address, you authorize Power Wellness to contact you regarding appointments, billing, patient services, treatment coordination, administrative matters, and other communications you have agreed to receive.
Standard message and data rates may apply. Email and ordinary text messaging may not always be fully secure. Sensitive clinical information should be communicated through the approved patient portal or another secure method when directed.
Marketing communications may be subject to separate consent and opt-out rights.
15. Privacy
Our collection, use, and disclosure of information are governed by our Privacy Policy and, where applicable, our HIPAA Notice of Privacy Practices.
Telehealth appointments, patient messages, billing information, and related health information may be protected by HIPAA and other applicable laws. HHS advises that covered healthcare providers use secure technologies and appropriate safeguards when delivering telehealth services.
16. Website Accounts and Security
You are responsible for maintaining the confidentiality of your login information and for activity occurring through your account.
You agree to notify Power Wellness promptly if you believe your account, password, payment information, or personal information has been accessed or used without authorization.
You may not attempt to gain unauthorized access to the website, patient portal, systems, records, or accounts belonging to another person.
17. Acceptable Use
You may not use our website or services to:
- Violate any applicable law or regulation.
- Submit false, fraudulent, threatening, abusive, or misleading information.
- Attempt to obtain prescriptions or controlled substances through deception.
- Interfere with the operation or security of our website or systems.
- Copy, scrape, reproduce, distribute, or commercially exploit website content without authorization.
- Upload malware or otherwise harm our website, systems, staff, providers, or users.
We may suspend or terminate website access for conduct that violates these Terms.
18. Intellectual Property

To the fullest extent permitted by law, the website and general website content are provided on an "as is" and "as available" basis.
Power Wellness does not warrant that the website will always be uninterrupted, error-free, completely secure, or free from harmful components.
Nothing in this section limits professional duties that cannot legally be waived or modifies any rights established through a separate provider-patient relationship.
21. Limitation of Liability
To the fullest extent permitted by law, Power Wellness and its owners, officers, employees, contractors, and affiliates will not be liable for indirect, incidental, special, consequential, exemplary, or punitive damages arising from the use of, or inability to use, the website.
This website limitation does not exclude liability that cannot lawfully be limited, including any liability governed by applicable healthcare or professional-liability laws.
22. Indemnification
You agree to indemnify and hold Power Wellness harmless from claims, damages, losses, liabilities, and expenses arising from your misuse of the website, violation of these Terms, fraudulent conduct, or infringement of another party's rights.
This section does not apply to the extent a claim results from Power Wellness's own negligence, professional obligations, or conduct for which indemnification cannot legally be required.
23. Suspension or Termination of Services
Power Wellness may suspend website access, administrative services, or membership benefits when these Terms, payment obligations, safety requirements, or applicable policies are violated.
Any termination of an established provider-patient relationship will be handled in accordance with applicable professional and legal requirements, including appropriate notice when required.
24. Governing Law
These Terms are governed by the laws of the Commonwealth of Pennsylvania, without regard to conflict-of-law principles.
Any legal action relating to website use or these Terms must be brought in a court of competent jurisdiction located in Pennsylvania, unless applicable law requires otherwise.
25. Changes to These Terms
Power Wellness may update these Terms periodically. Updated Terms will be posted on this page with a revised effective date.
Continued use of the website after updated Terms are posted constitutes acceptance of the revised Terms, except where additional notice or affirmative consent is required by law.
26. Severability
If any part of these Terms is determined to be invalid or unenforceable, the remaining provisions will remain in effect.
27. Entire Agreement
These Terms, together with the Privacy Policy and any separately executed patient, membership, payment, treatment, telehealth, or informed-consent agreements, constitute the applicable agreement between you and Power Wellness regarding your use of the website and services.
A separately signed agreement will control if it contains terms that specifically conflict with these general website Terms.
